For Benefited Employees Only Termination of University Employment NotificationThis notification is required if a benefited employee is leaving the employment of the University. Please do not use this form for student employees, casual labor employees or if a regular employee making an internal transfer. Please fill out this form so the appropriate personnel are informed in advance of a change in employment status. For security, coordination, and informational purposes; it is best if the appropriate personnel across the university are informed as soon as possible of the impending departure of an employee. Budget Services and Human Resources will still require a PAR to be filled out and submitted as before. |
